The Connected2Care Project

The Connected2Care Project at NYU Langone’s Center for Healthful Behavior Change aims to translate evidence-based research into practice to better manage chronic diseases. We offer programs that bridge community health promotion with the healthcare delivery system to enhance the quality of care and improve clinical and behavioral outcomes in diverse patient populations.

Our deeply experienced project team, directed by Antoinette M. Schoenthaler, EdD, is currently enrolling participants in the i-Matter and ALTA studies.

i-Matter Study: Investigating an mHealth Texting Tool for Embedding Patient-Reported Data into Diabetes Management

i-Matter is designed to engage patients to take an active role in managing their type 2 diabetes by helping them rate and record their health behaviors and quality of life in real time on their mobile devices and making it actionable in personalized reports.

ALTA: Advancing Long-Term Improvements in Hypertension Outcomes Through a Team-Based Care Approach

Through a structured partnership model, ALTA is designed to ensure that primary care practices have the latest evidence to improve medication adherence and blood pressure control in their practice, and have the skills and resources to use the evidence to help their patients live healthier and longer lives.
