The NYU School of Medicine, Student Health Service is legally and ethically obligated to protect every patient’s right to privacy.  Health records are strictly confidential and maintained in compliance with federal and New York State laws including the Health Insurance Portability and Accountability Act (HIPAA). The Student Health Service will not release medical information to anyone, including family, parents, NYU faculty/ staff or outside agencies without the written authorization of  the patient, unless required by law.  In a medical emergency only relevant health information will be released to another health care provider.

If a patient wishes to release his/her records to another provider, authorization forms may be obtained from the Student Health Service office during usual business hours.  If you have any questions regarding the release of health information, please call 212-263-5489.
