United Healthcare Insurance

The United Healthcare plan coverage is from September 1, 2008 to August 31, 2009. The cost is as follows:

$3,500- for individual (student) coverage per academic year

$9,000- for family coverage per academic year

The 2008-2009United Healthcare Enrollment and Student Health Insurance Waiver forms cannot be e-mailed back to us, but we provide them here for your convenience so that you can print them out (we have also posted instructions for completing the forms). Completed forms can be dropped off in the tray next to the Registrar's counter; given to Joseph Oppedisano, Sue Kaelin, or Ivie Martinez; or faxed to (212) 263-0520.

Please note: Waiver forms will not be processed with the Bursar until late August. Tuition and fee bills will not reflect the credit for health insurance until the start of the academic semester (September).
