United Healthcare Insurance

The United Healthcare plan coverage is from September 1, 2009 to August 31, 2010. The cost is as follows:

$3,850- for individual (student) coverage per academic year

$9,900- for family coverage per academic year

The 2009-2010 United Healthcare Enrollment and Student Health Insurance Waiver forms cannot be e-mailed back to us, but we provide them here for your convenience so that you can print them out (we have also posted instructions for completing the forms). Completed forms can be dropped off in the tray next to the Registrar's counter, given to Joseph Oppedisano, Sue Kaelin, or Ivie Martinez; or faxed to (212) 263-0520.

If you wish to enroll or waive the United Healthcare Insurance during the semester, you must submit a Qualifying Event form with your enrollment/waiver form. You must also submit proof of the qualifying event (copy of marriage certificate, letter denoting loss of insurance, etc) within 30 days of the event. Please note: the cost of insurance will not be pro-rated; you will be required to pay the full cost for insurance for the semester regardless of when you enroll.

Please note: Waiver forms will not be processed with the Bursar until late August. Tuition and fee bills will not reflect the credit for health insurance until the start of the academic semester (September).
