Referral

Getting Started

We require that you be referred to our center through your primary care physician. A downloadable referral form is available for you to bring to your doctor. When the form is returned to our offices, our coordinator will contact you about the referral and gather any additional information.

Once an appointment is scheduled, the coordinator will send you a packet of sleep logs and questionnaires. You will need to bring these forms, plus any insurance information, with you for your first appointment. The first appointment will not include sleep testing.

Insurance

The Center accepts most major insurance carriers, but it is best to check with us if your sleep evaluation is covered under you plan. Please contact Yvette or Barbara at (212) 263-8423 with questions regarding insurance or referrals.

Evaluation and Treatment

The center will provide consultation and evaluation and suggest an appropriate course of treatment to the referring or primary care physician who typically provides follow up care for the patient. A complete diagnosis may include one to four individual sleep studies.

An overall plan of care is developed for each patient, taking into account the psychological, physical, social, educational, and cultural implications of each patient's condition. Quality of care is maintained through the communication and/or implementation of the treatment plan by each discipline with the referring physician.
