Description and Phases of the Program
Post-Discharge, Maintenance/Follow-Up Therapy (Third Phase)
At the successful completion of the Guided Work Trials patients are helped with finding work in the community, commensurate with current capabilities. Then, follow-up or maintenance therapy is prescribed, as needed.
Post-discharge follow-up and/or maintenance treatment is
prescribed according to the individual patient's needs. The
number of treatment hours per week varies from individual
to individual.
