Description:
The surgical sub-internship in surgery is meant to give the student who is interested in surgery a more in depth experience in surgery. The student will take on the responsibilities of an intern. In this capacity, he or she will see consults in the emergency room and on the floor, manage patients that have been admitted to the service, and assist in the operating room. In addition, the student will be responsible for a short project, either a presentation or a short paper on a relevant topic.
Objectives of the Elective:
This rotation introduces the student to general surgical principles including pre-op evaluation, intra-op decision making and post-op care. Students will deal with surgical illness, surgical critical care and non-operative care of the critically ill. Students should become “experts on the abdomen.”
Key Responsibilities of the student while on Elective:
- Students will follow patients and provide care, presenting on attending
rounds and assisting in the operating room.
- Students have night call and participate in evaluation of patients.
- Students attend all surgery conferences.
Didactic Program:
Rounds and clinics are scheduled for each location.
Method of Evaluation:
Students are evaluated by the housestaff and those attendings who interact most closely with them.

Title: Pediatric Surgery Subinternship

Description:
The combined Pediatric Surgery Service at Tisch and Bellevue Hospitals offers a subinternship for a fourth year medical student. The subintern functions as a member of the junior house staff, and will work with the junior residents on the Pediatric Surgery Service under the supervision of the pediatric surgeons and the Chief Surgical Resident on the service. Close co-ordination with the Department of Pediatrics permits an unusual opportunity to study the surgical aspects of pediatric care from neonatal through the teenage years.
Objectives of the Elective:
This elective introduces the student to pre- and post-operative pediatric surgical care, a management of simple but common pediatric surgical problems, e.g. hernias, undescended testes, pyloric stenosis, intussusception. Students will learn basic knowledge of major congenital anomalies, e.g. esophageal atresia, intestinal atresia, ompahlocele and gastrocechesis, Hirschsprung’s disease and imperferatcious.
Key Responsibilities of the student while on Elective:
- Attendance at daily morning rounds.
- Participation in the operating room.
- Presence at all pediatric surgical and related conferences with possible student presentations.

Method of Evaluation:
Students will be evaluated by attending with input from surgical house staff rotating through pediatric surgery. There will be no exams.

Title: Plastic/Reconstructive Surgery
Preceptor: Pierre Saadeh, M.D.

Description:
The Institute of Reconstructive Plastic Surgery offers a comprehensive elective in general plastic surgery. The student is exposed to the entire spectrum of the specialty including craniofacial surgery, cleft lip and palate surgery, microsurgery in trauma and post-cancer ablation reconstruction, hand surgery, maxillofacial trauma surgery, trunk and lower extremity reconstruction, breast reconstruction and cosmetic surgery. Clinical research projects are always available for student participation. Basic science research opportunities are also available in cranial suture biology, wound healing, craniofacial skeletal distraction, microsurgery, craniofacial growth and organogenesis. The student is encouraged to individualize the experience for his/her own needs. A full rotation with emphasis on hand surgery for the student with such an interest can be arranged. A typical rotation will allow the student to spend 2-3 weeks on the Plastic Surgery service at Tisch Hospital/NYU and 1-2 weeks on the Plastic and Hand surgery services at Bellevue.
Objectives of the Elective:
This elective will expose the student, in a very short time, to the full spectrum of plastic surgery. The student will learn the approach to the diagnosis and treatment of congenital defects and anomalies and learn the alternative methods and indications for the coverage of acquired defects and wounds. By the end of the four-week rotation, the student will better understand the problem-solving approach that is at the core of reconstructive plastic surgery.
Key Responsibilities of the student while on Elective:
- The student will be an integral part of the plastic surgery team.
- The student will make morning rounds with residents.
- The student will scrub and assist in the operating room daily
- The student will assist in the evaluation of emergency room patients.
- Students will attend conferences.

Title: Surgical Oncology Subinternship

Description:
Students in surgical oncology participate in all aspects of in-patient and out patient care in the diagnosis and management of surgical oncology and surgical endocrinologic patients. Management of breast cancer, malignant melanoma, surgical thyroid and parathyroid disease and other soft-tissue neoplasms are emphasized.
Objectives of the Elective:
The elective is for students who have completed the required Surgical Clerkship. The elective gives students an opportunity to concentrate on those areas of oncology managed by the Division of Surgical Oncology. These include clinical and research efforts in the treatment of malignant melanoma, carcinoma of the breast, soft tissue neoplasms, head and neck tumors, and surgical endocrinology. Students participate in the daily management of patients with these clinical problems, thereby acquiring an in-depth knowledge of the issues in diagnosis and treatment.
Key Responsibilities of the student while on Elective:
- The student will participate in the hospital management of patients as well as in the office setting at the time of evaluation and follow-up, at the time of admission to the hospital for surgical treatment, and in the Operating Room.
- The student will attend oncologic conferences of the Department of Surgery and related oncologic conferences of other departments that are multidisciplinary.
- The student will function as a sub-intern in providing care under the supervision of the faculty and a Senior Surgical Resident assigned to the Surgical Oncology Service.
Didactic Program:
A specific reading program will be provided for the students and there will be weekly meetings with senior surgical faculty members to discuss issues in Surgical Oncology and specific clinically related problems regarding patients encountered during that week.
Daily teaching rounds will be held with the student.
Method of Evaluation:
The students will be evaluated on their performance by both the senior House Officer and the faculty. The student is encouraged as well to participate in ongoing surgical-oncology-related research which can be initiated during this rotation and pursued during the subsequent academic year(s). Evaluation will be based upon the level of knowledge displayed by the student on a daily basis, the depth of his or her reading, and the clinical performance as evaluated by the Surgical Housestaff and faculty.

Title: Transplantation Surgery

Description:
Subinterns on the transplant service actively participate in all aspects of care including pre-operative assessment of candidates for liver, kidney or pancreas transplantation. Students will assist in the operating room, and in post-operative management and immunosuppression. The attending transplantation surgeons are committed to maximizing the learning opportunities for subinterns on the service. Principles of transplantation biology, including immunology are emphasized.
Objectives of the Elective:
Introduction of the student to transplantation by means of direct clinical involvement with all aspects of pre-and-post-transplant care. They will become familiar with the "language of transplantation" and be introduced to operative techniques including donor procurement and recipient surgery.
Key Responsibilities of the student while on Elective:
The subintern will be an important part of the transplant team. They will attend:
1. All transplant operations
2. Daily rounds
3. Pre-and-post transplant clinic
4. All surgery conferences attended by the residents
Didactic Program:
Daily rounds include abundant teaching. Weekly grand rounds and M & M conferences are mandatory.
Method of Evaluation:
Involvement and enthusiasm are prime determinants of a successful rotation.

Title: Trauma Surgery

Description:
The Trauma Service at Bellevue Hospital offers a subinternship in Trauma for fourth year medical students. The subintern functions as a member of the junior house staff and assumes first call responsibility for approximately 10 patients, including night rotation under the supervision of senior residents and the Trauma attending staff. Between 100 and 150 patients are admitted to the Trauma Service each month for blunt and penetrating soft tissue trauma. The service offers an unusual opportunity to study the management of the patient with trauma to multiple organ systems, problems of massive fluid replacement and transfusion, and impairment of respiratory, renal and cardiac function. Close coordination with the Department of Rehabilitation, Plastic Surgery and Orthopedics help to broaden the experience on the Trauma service. Attending rounds and conferences are held weekly. A member of the faculty serves as a preceptor for each subintern.
Objectives of the Elective:
This elective will expose the student to the early evaluation of the trauma patient, to gain a familiarity with trauma surgery and critical care in the pre and post-operative periods.
Key Responsibilities of the student while on Elective:
Students are expected to function as integral members of the trauma team under the constant supervision of Senior Residents. Patients are followed from the Emergency Room to the OR and then discharge.
Didactic Program:
Didactics in trauma are constantly taking place by virtue of the nature of the service.
Method of Evaluation:
Student evaluation will be based on a combination of the Chief Resident and Attending evaluation.

Title: Vascular Surgery Subinternship
Preceptor: Patrick J. Lamparello, M.D.

Description:
The subintern in vascular surgery participates in the care of patients with arterial and venous disease on the vascular surgery service at Tisch and Bellevue Hospitals. Outpatients are seen with diagnostic problems and for post-operative follow-up. In-hospital activities involve patients with a wide variety of arterial and venous syndromes. In addition to daily ward rounds and the weekly Vascular Surgery Conference, the student participates with the house staff in pre-operative and post-operative care and assists in surgery. There are ample opportunities in the OR, office practice, and rounds for student-faculty participation.
Objectives of the Elective:
The student will become exposed to all of the common problems associated with vascular disease, including arterial insufficiency, both in the lower extremities and in the carotid arteries, aneurysmal disease of the aorta and other vessels, venous insufficiency and in some cases, lymphatic disorders. The goal is to have the student become familiar with the use of noninvasive diagnostic tests, interventional radiology and surgery for the management of patients with these problems.
Key Responsibilities of the student while Elective:
- The student will work closely with the housestaff in the management of vascular patients in the hospital. This would include histories and physicals, preoperative and postoperative assessment, as well as daily rounds.
- The student will spend more time in the Operating Room than the junior housestaff officers, giving them the opportunity to see the pathology firsthand.
- Students will see outpatients in the office area with Dr. Lamparello.
- In general, the students take night call twice a week with a senior
housestaff officer.
Didactic Program:
Vascular Service has a weekly Tuesday morning conference geared toward students and housestaff officers.
Method of Evaluation:
The students are graded according to their level of participation, their involvement in the rounds and conferences and office hours, their knowledge of medicine and surgery and their potential as a future housestaff officer.
