Palliative Care

Description:
The students will participate fully in the clinical, educational,
and research activities of the Palliative Care Program. Students will participate
in regularly scheduled clinical rounds with the Palliative Care Team, which
includes social worker, R.N., chaplain, and physician. The students will benefit
from observing the physician-patient interaction in this mentoring setting.
Family meetings, the number of which will vary from week to week will be an
important aspect of the student's experience. The weekly patient management
conference involves the expanded Palliative Care support team to include psychiatry/psychology,
pain management, and dietary. In addition, students will participate in the
weekly conference with the Pain Management Service. There is also a monthly
Palliative Care education conference. One week of the elective will be spent
at Calvary Hospital. The student will also spend one day making home visits
with Cabrini Hospice.
Objectives of the elective:
During the elective, the students should
develop an appreciation and understanding of Palliative Medicine and its important
place in the continuum of care of patients with a life threatening illness.
Key Responsibilities of the student while on Elective:
- The student is expected to attend regularly scheduled clinical rounds with
the Palliative Care Team as well as appropriate family meetings.
- The student is expected to be present for all pertinent conferences.
Didactic Program:
Conferences include the monthly Palliative Care education
conference and Pain Management conferences.
Method of Evaluation:
The students will be evaluated by the faculty
preceptor.

Title: Pulmonary Consultation

Prerequisites: Medicine Clerkship
Description:
The student will take part in daily pulmonary consultation
rounds. Students will, along with senior pulmonary fellows, be able to evaluate
and examine patients with a wide spectrum of pulmonary diseases. Students will
also be encouraged to participate in multidisciplinary conferences (oncologic,
thoracic surgery, chest radiology, and pathology) as well as observe pulmonary
procedures including bronchoscopy and transthoracic lung biopsy.
Objectives of the Elective
The students will learn the principles of
pulmonary medicine. They will learn to generate a differential diagnosis and
management regimens for patients with pulmonary disorders.
Key Responsibilities of the students while on Elective
Students will
attend daily pulmonary consultation rounds with the first and second year pulmonary
consult fellows. The students will see and present various cases to the pulmonary
consult attending.
Didactic Program:
Attending conferences are held daily as well as a
variety of weekly multidisciplinary conferences which the student is expected
to attend.
Methods of Evaluation:
Students are evaluated by attendings assigned
to work with students who then present their findings to the preceptor who
is responsible for final evaluation.

Title: Psychosocial Medicine

Description:
This elective focuses on the medical interview and the
doctor-patient relationship, two core aspects of doctoring that tend to be
under-emphasized in medical school and residency training. As well, we seek
to acquaint students with the broad range of psychosocial issues relevant to
medicine, and to foster the development of a "biopsychosocial" understanding
of illness and treatment. The elective, which is taught by experienced Primary
Care faculty and fellows, is individualized and student-centered, and utilizes
a variety of active and participating teaching approaches.
Objectives of the elective:
Our specific objectives are that the student
will: (1) Understand the structure and function of the medical interview; (2)
demonstrate proficiency in basic skills of the medical interview; (3) understand
the biopsychosocial approach to diagnosis and care; (4) attain sound knowledge
of the important psychosocial problems in general medicine (depression, anxiety,
somatiziation, reactions to illness, etc.).
Key Responsibilities of the student while on Elective:
- The student will meet with the faculty preceptor four times per week for
two hours.
- The student will be expected to interview several new patients (generally
inpatients) each week, and to follow 2-4 patients during the course of their
hospitalization.
- The student will make several informal presentations on topics of their
choice.
Didactic Program:
Most teaching is in tutorials of one faculty or fellow
preceptor with 1-2 students. Teaching is primarily "hands-on" (i.e.
observation and discussions of patient interviews, supervision of on-going
cases) and small group seminar/discussions of selected topics.
Method of Evaluation:
Students will be evaluated based on: (1) direct
observation of interviewing and related skills; (2) assessment of master of
knowledge in tutorials and small group seminars; and (3) observation of student
presentations.
