Description:
The Department of Anesthesiology offers a four week elective for a third
or fourth year student who has completed both Medicine and Surgery Clerkships.
This comprehensive elective introduces the student to preoperative evaluation,
intraoperative management and postoperative routine and critical care. Emphasis
is placed on the scientific foundations of anesthesia, including organ-system
pathophysiology, autonomic and anesthetic pharmacology, and hemodynamic monitoring.
Morning case conferences and afternoon topical lectures will supplement operating
room patient care responsibilities. In the operating room, there is a one-to-one
teaching by both anesthesia residents and faculty. The student may elect to
do a one week rotation in a specialty field of anesthesia, e.g. obstetrics,
neurosurgery, cardiothoracic or pediatrics. On-call responsibilities at Bellevue
Hospital will be one evening per week during which time the student will work
with the Bellevue Team Captain.
Objectives of the Elective:
This elective introduces the student to preoperative evaluation, intraoperative
management and postoperative routine critical care. The student will obtain
a basic understanding of anesthesia, including organ-system pathophysiology,
autonomic and anesthetic pharmacology, and hemodynamic monitoring.
Key Responsibilities of the student while on Elective:
- To perform a preoperative anesthetic evaluation with the assistance of
the resident.
- To understand the patient's medical problems, the anesthetic implications
and develop a rational anesthetic plan. A keyword and topic list is distributed
and indicates the minimum topics which should be reviewed.
- To assist the anesthesia resident in setting up the operating room in the
morning and with the cases during the day.
- To attend morning teaching conferences, afternoon lectures, and departmental
conferences.
Didactic Program:
In general, the program is as follows:
7:15 a.m. Anesthesia Management Conference at Bellevue
8:00 a.m. to 1:00 p.m. in the Operating Room
1:00 p.m. Lecture/Rounds at Bellevue
4:00 p.m. Preoperative/postoperative patient evaluations.
Tuesday 5:00 p.m. Departmental Grand Rounds
Tuesday 6:00 p.m. Topical Review
Wednesday 7:00 a.m. Departmental Lecture
The didactic program varies somewhat depending on the hospital where the
student is rotating.
Method of Evaluation:
Evaluation is based
on completed attending evaluation forms and oral evaluation with topics to
be drawn from the list of Key Topics and the Suggested Reading List.

Title: Anesthesiology

Description:
The student will take part in
anesthetic management of patients for orthopaedic surgery. Emphasis will be
placed on basic scientific and clinical aspects. Endotracheal intubation, regional
anesthesia, and hypotensive anesthesia will be taught. Monitoring for spinal
cord surgery will be used to demonstrate the use of this new concept in clinical
medicine. All up-to-date methods of monitoring including pulmonary artery catheterization,
CVP, arterial line, SaO2, and CO2 analyzers are available as educational tools.
Objectives of the Elective:
The student will
learn the principles of general and regional anesthesia,endotracheal intubation
and placement of lines. This is accomplished by preoperative evaluation, intraoperative
management, and postoperative follow up.
Key Responsibilities of the student while on Elective:
- Students will attend the preoperative conference where cases are seen,
presented and discussed. Anesthetic considerations related to specific medical
problems are emphasized.
- The student will prepare presentations at the weekly Grand Rounds where
specific problems are discussed in depth.
Didactic Program:
Attending conferences are
held daily with the student using the case method to help the student learn
the basic and clinical principles in anesthesia.
Method of Evaluation:
Students are evaluated
by attendings assigned to work with student who then present their findings
to the preceptor who is responsible for final evaluation.

Title: Critical Care

Description:
The Department of Anesthesiology
offers a comprehensive four week elective in critical care at Bellevue Hospital.
During the rotation, the critical care student experiences preoperative, intraoperative
and postoperative critical care in a diverse patient population which affords
a large variety of clinical pathology as well as the excitement of acute critical
care in a predominantly young trauma population. Continuity of care is stressed
by following critically ill patients from the operating room to the recovery
room and then to the surgical ICU's. The student assists in the intraoperative
management and resuscitation of critically ill patients. In the recovery room
the student learns about postoperative care and works with the recovery room
attending and the critical care resident. In the surgical ICU's, the critical
care student is a member of a multidisciplinary Critical Care Consult Team,
whose members include a critical care attending, recovery room attending and
a pulmonary/critical care fellow. During lectures and rounds, an emphasis is
placed on the basic principles of shock, resuscitation, organ-system pathophysiology,
multiple organ dysfunction and the fundamentals of hemodynamic and physiologic
monitoring. On-call responsibilities at Bellevue Hospital will be one evening
per week during which time the student will work with the Bellevue Team Captain.

of this elective is to provide the student with an introduction to critical
care in the surgical, trauma and neurosurgical critical care unit. The student
will obtain a basic understanding of fundamental concepts of critical care
management through rounds, case discussion, reading and lectures. During lectures
and rounds, an emphasis is placed on the basic principles of shock, resuscitation,
organ-system pathophysiology, multiple organ dysfunction and the fundamentals
of hemodynamic and physiologic monitoring. On-call responsibilities at Bellevue
Hospital will be one evening per week during which time the student will work
with the Bellevue Team Captain.
Key Responsibilities of the student while on Elective:
- To perform a patient examination and evaluation in the ICU, describing
the problems of a critically ill patient and to determine the initial steps
of action to correct important problems.
- To assist the critical care resident patient care.
- To attend the morning and afternoon critical care rounds, afternoon didactic
lecture, and departmental conferences.
- Keyword and topic list, which will be distributed, will indicate the minimum
topics which should be reviewed.
