To save time, you may download, print and fill out the appropriate forms at home, and bring them with you on the day of your appointment.
CT
If you are having a CT scan (all types of CT scans except for Dental), please print and fill out these forms.
If you are having a Dental CT scan, please print and fill out these forms.
Ultrasound and X-Ray Walk-Ins
If you are having an Ultrasound or are coming as a walk-in for X-Rays, please print and fill out these forms.
