Research Summary

Dorsal wrist extensor tenosynovitis is exceedingly common in patients with rheumatoid arthritis often causing pain, limited motion, and tendon rupture. Early surgical tenosynovectomy is usually recommended when medical management fails. In the non-rheumatoid population, fourth extensor compartment proliferative tenosynovitis is very rare. Characteristic in the physical examination is a tender dorsal mass and severe limitation of active wrist extension while the fingers are concurrently extended due to interference of proximal tendon excursion into the fourth extensor compartment. Active wrist extension is improved with fully flexing the fingers in to a fist which draws the affected tendons more distally. A review of our case series demonstrates a distinct pathological condition of exuberant proliferative, adherent tenosynovitis of the extensor digitorum communis tendons blocking proximal excursion of the tendons and thereby causing pain and limited active wrist extension. We will delineate the histological features of this tenosynovium and compare and contrast it to that seen in the more typical rheumatoid extensor tenosynovitis. We intend to describe this clinical entity which is uncommon but not rare.
