Expansion of the Rusk Institute
The Rusk Institute Rehabilitation Network, with its two Inpatient Rehabilitation sites and three Outpatient centers, has expanded to include seven Sub Acute/ Skilled Nursing Facilities. January 1st marked the official beginning of an affiliation between the Rusk Institute of Rehabilitation Medicine and the seven sub acute members. Rusk Institute Rehabilitation Network now offers patient care and rehabilitation through all phases of recovery.
The newest members of the Rusk Institute Rehabilitation Network are located throughout the boroughs and in Westchester. The members include: Cabrini Center for Nursing and Rehabilitation and Village Nursing Home in Manhattan, Metropolitan Jewish Geriatric Center and Shorefront Jewish Geriatric Center in Brooklyn, Margaret Tietz Nursing and Rehabilitation Center in Queens, Morningside House in the Bronx and St. Cabrini Nursing Home in Dobbs Ferry.
Each of the Sub Acute (SA)/Skilled Nursing Facilities (SNF) maintains a distinct rehabilitation unit. Individualized care plans are developed with the patient and family by an interdisciplinary team. The team includes: Medicine, Nursing, Occupational, Physical, Speech/Swallow and Recreation therapies and Social Work. Comprehensive discharge planning is also provided by each facility to ensure a smooth transition home with continuity in the patient’s rehabilitation.
The Office of Continuing Care is collaborating with clinicians throughout the Network at NYUMC, NYUHJD, Rusk and the seven SA/SNFs to expand clinical programs, facilitate continuity of care and further enhance the quality of services rendered at member facilities. Planning for specialty programs at the SA/SNFs in the areas of Cardiopulmonary and Stroke Care Management are well underway. In addition, ongoing clinical seminars and in services at Tisch and Rusk are open to clinicians throughout the Network.
