History of the Lower New York Consortium

1991

LNYC is established with a Ryan White CARE Act Title III Early Intervention grant, enabling the family-centered model of care provided in the Bellevue Pediatric Diseases Clinic to be exported to St. Vincent’s Catholic Medical Center – Manhattan and Richmond University Medical Center.

1994

LNYC receives Ryan White Title IV funding and is able to:

  • develop an infrastructure for program management, development and evaluation
  • support community case management at two community AIDS organizations: AIDS Service Center NYC and Community Health Action of Staten Island
  • develop a mechanism of care coordination between and among community agencies and hospitals
  • create an outreach program and Clinical Services for adolescents

1995 to 2003

LNYC receives Ryan White Title I funds from the New York State AIDS Institute which fund prenatal programs for HIV counseling and testing, counseling about AZT, and allow for partnerships with the pediatric infectious diseases programs for the referral to and care of HIV-exposed newborns. Title IV also supports outreach to minority youth at Ryan-NENA Community Health Center and at Community Health Action of Staten Island.

Fall 2003

LNYC receives New York State AIDS Institute support to create a Youth Specialized Care Center at Bellevue Hospital and a Youth Access Program at Community Health Action of Staten Island.

Fall 2004

LNYC receives Title IV award for a Youth Program to further expand its targeted outreach program and clinical and social services for infected youth.

Today

LNYC currently provides a substantial amount of support for the clinical and social work staff that offers family-centered services at member sites. Additionally, under the auspices of its Case Management Coordinator, the Consortium sponsors monthly meetings for social workers and community case managers from LNYC member and other community service providers in lower Manhattan and Staten Island. Other participating agencies include:

  • The Henry Street Community Consultation Center
  • Selfhelp – a homecare agency
  • Betances Community Health Center
  • The Chinese American Planning Council
  • Chinese-American  Planning Council
  • The Family Center
