In collaboration with the medical / nursing staff at Bellevue, social work provides care coordination services for children with special health care needs, including referrals for concrete services, and interfacing with schools, community-based organizations, social service, child protection and foster care agencies.

Social work staff also participates in medical care coordination by ensuring medical follow up for children with special health care needs, and by assisting these patients and families to obtain access to needed supplies, equipment and medications. They also provide supportive individual and group counseling when indicated.
