Office visits are by appointment only. Please plan your visit in advance so that we may accommodate your preferred time. Emergency visits can often be accomodated by your physician or one of his or her colleagues. Please call the office to schedule.
If you must cancel an appointment, please do so at least 24 hours before your appointment. This allows us to offer your appointment to someone in need of an immediate appointment.
Calls are forwarded to our answering service during non-office hours.
When calling to schedule an initial appointment, please have the following information available:
- Patient’s Full Name
- Date of Birth
- Home Phone Number
- Work Phone Number
- Cell Phone Number
- Social Security Number
- Home Address (city, state and zip code)
- Diagnosis
- Name of Referring Physician
- Name of Primary Insurance and Policy Number
- Name of Secondary Insurance and Policy Number
- Name of the Insured Party
