One non-categorical (single year contract) resident is accepted per year to function as a junior resident at the Manhattan VA hospital. Applicants are often foreign medical school graduates or those who failed to match the previous year.

Externs gain experience in patient care and surgical technique. Because of extensive nurse practitioner support and the small number of emergencies in the VA systems, non-educational 'scut' work is minimal.

The majority of externs receive a strong letter of recommendation and subsequently match in neurosurgery programs across the country. Occasionally, externs remain for a second year to work at Tisch and Bellevue hospitals.
