Financial Information

Because infertility treatment may be a progressive pathway utilizing a variety of modalities, it is important for you to research your specific insurance coverage prior to treatment. Our onsite Finance Department is available to help review fees and provide information and assistance whenever possible. However, it is of the utmost importance that you fully understand the financial commitment you are making once you decide to proceed with treatment.

Physician Fees

The NYU Fertility Center offers comprehensive, state-of-the-art infertility treatments at competitive rates. All fees incurred for services rendered by consulting physician are due at the time of service. This includes consultation and return office visits, ultrasounds, office procedures and HSG.

In the case of in vitro fertilization, payment is required at Day 2 or Day 3 of the IVF cycle. For donor egg cycles, payment is required when the recipient is matched to a donor. In the event of non-payment, the cycle may be canceled.

We accept cash, checks, Visa, MasterCard and American Express.

You will receive a bill that can be submitted as necessary to your insurance carrier. It will include your diagnosis, as determined by your physician, and procedure codes for the services rendered.

Fees may also be incurred for outside providers while you are under treatment at NYU Fertility Center. These fees include, but are not limited to, laboratory testing, radiologists', pathologists', anesthesia, pharmaceutical or NYU Medical Center facility charges.

Insurance Information

NYU Fertility Center policy requires all payments to be made at the time of service. If you have insurance, you are responsible for obtaining reimbursement for services rendered from your insurer directly.

Many health insurers have an online process that allows for verification of coverage for infertility services. We also suggest that you contact your insurer’s customer service call center. Please note that even if you have an infertility benefit, you may still be required to obtain prior authorization before starting therapy.

Some insurance plans have lifetime maximums for infertility treatment and require prior authorization/pre-certification and/or registration with infertility case management once it is determined that treatment is appropriate. Although our onsite Finance Department will provide assistance whenever possible, it is your responsibility to obtain proper authorization and/or to register with the appropriate case management team at your insurance carrier.

As is insurance carriers' policy, pre-authorization does not guarantee payment. Benefits may be denied or only partial payments received based upon the status of your benefit at the time the claim is processed. Patients covered under any insurance plan are responsible for payment of co-pays, co-insurance, deductibles, excluded or non-covered services/procedures at the time of service or upon notification. The patient must provide full and complete information regarding their primary and secondary insurers, and follow the guidelines established by the carriers for obtaining benefits.

You should be aware that insurance plans written in NY are mandated by law to provide coverage for infertility treatment and diagnosis, however, IVF is excluded from the mandate.

Major and minor surgeries must be preauthorized with your insurance carrier. In most cases, prior authorization for surgery will be obtained by the administrative assistant of your consulting physician. A deposit for surgery will be required at the time the surgery is scheduled. The patient is reminded that preauthorization does not guarantee full or any payment of the physician's fee, since payment determination is made by the carrier at the time the claim is received. You are responsible for all outstanding charges (deductible/co-payment/denial) once the insurer has determined its responsibility for the fee(s).

NYU Fertility Center currently participates in United HealthCare's Empire Plan as a Center of Excellence for infertility treatment and United HealthCare's PPO plan for the employees of the NYU School of Medicine, in addition to Aetna and United Resource Network.
