6. What can the Palliative Care Consult Service offer and how can I make a Palliative Care Referral for my patient?

• The Palliative Care Team offers expertise in symptom management, assisting patients and families in setting goals of care, and discharge planning for patients facing a life threatening illness or at end of life.
• Palliative Care consults can be obtained by contacting the Palliative Care Service at 30900 or speaking to one of the Palliative Care Consult team members: Joe Lowy MD 685-6002; Catherine White, APN, beeper 1418; Stuart Kaufer, CSW, beeper 1207.
