Emergency Visits
The Immediate Care Center (I-Care) at the Hospital for Joint
Diseases is always open and available for all patients with urgent spinal
conditions if a same-day appointment cannot be made or for problems outside
of open hours of the Spine Center, including weekends. The I-Care is
located at the first floor of the Hospital, 301 East 17th Street (northeast
corner of 2nd Avenue).Direct phone inquiries to 212) 598-7600.
Scheduling Appointments
Appointments are required for all office visits. You may contact
us by phone for an office visit, and we will schedule an appointment
for you as quickly as possible. At the time of the call, you will be
asked to describe your symptoms and/or medical problem. Medical insurance
information must also be provided at this time. Tel: (212) 598-6625.
Fax: (212) 598-6723. Monday through Friday, office staff is available
from 9:00 AM until 4:00 PM.
First Visits
Please arrive approximately 30 minutes in advance of your appointment.
This is necessary in order to complete the necessary medical and insurance
forms. You can save time by downloading a copy of our First Visit New
Patient Form and filling it out prior to your visit, bringing it with
you and faxing us a copy prior to the day of your appointment (click
here to download First Visit New Patient Form). Please bring any
and all pertinent test results with you, including medical reports and
films of all X-ray studies and scans. Also bring a list of all current
medications you are taking and a copy of any previous spinal operative
reports (if you have had prior spinal surgery). Remember, outpatient
office visits are held at the Spine Center outpatient facility, located
within the Orthopaedic Private Practice Facility. The entrance is near
the southwest corner of 2nd Avenue and 18th Street (240 East 18th St).
Canceling Appointments
Please call at least 24-hours ahead to cancel any scheduled appointment.
There is a waiting list for appointments and we need to notify other patients
that an opening has occurred. We will be happy to reschedule your appointment.
Appointments should not be made for nonmedical issues such as obtaining copies of office notes and for requesting letters. Phoning or faxing the office your request is sufficient.
