NYU faces W-9 form

NYU faces will host Art Day 2008 at The Children's Museum of Art on Saturday June 7th, 2008 from 9:00am to 11:30am. This exciting forum will give both you and your child access to the creative tools that promote self-expression and esteem through art. This event is open to pediatric patients ages 3 to 12 years old. Note: Due to space constraints, we ask that you limit your rsvp to 2 children and 1 adult. Summer Camp Scholarship: NYU faces is once again proud to offer children affected by epilepsy the opportunity to enjoy a wonderful summer camp experience! We encourage families with a demonstrated financial need to apply for a select number of scholarships towards tuition at camps around the country. The list of approved summer camps and a downloadable application form can be found on the Summer Camps page. Applications must be received by June 1, 2008 for consideration.
