1. The following areas must be inspected a minimum of semiannually by at
least two members of the IACUC:
a. Animal facilities managed by that DLAR
b. Satellite facilities (animals held > 24 hours) approved
by the IACUC
c. Study areas (animals held > 12 hours) approved by the
IACUC
d. Laboratories or animal activity areas where "...experiments
inclusive of surgical manipulation." are performed.
2. At least two members of the IACUC shall review the animals and the animal
activities of investigators in those facilities at least semiannually.
3. Remaining animal activity areas (laboratories, imaging areas, etc.)
a. At least one member of the IACUC (an agent of the IACUC)
should visit those remaining areas at least quarterly
b. Examples of activities, if applicable, that can be assessed:

Is appropriate equipment available to perform
euthanasia? |

Is C02 and a chamber available? |

If a guillotine is present, has it been maintained properly? |

If present, is C02 tank(s) properly secured? |

Are animal activity chambers/enclosures kept properly
sanitized? |

Are controlled substances in-date, properly stored, and
accounted for? |

Are sharps management practices appropriate?

Is appropriate signage posted at area entry? |

Is appropriate PPE available for species/activities present
in the area? |
| xii. |
Are workers at potential risk due to animal exposure enrolled
in occupation Health and Safety Program? |
| xiii. |
Is there a copy of the protocol(s) active in that lab
posted or available nearby? |
| xiv. |
Are personnel performing animal activities conversant
with content of the protocol(s)? |
| xv. |
Is SOP on file describing receipt and disposition of animals
and transport containers? |
c. Findings are to be reported to the IACUC and corrective actions, including
both plans and schedules, are to be reflected in the IACUC's next semiannual
report. The correction of those findings shall be tracked just as deficiencies
identified during semiannual reviews.
