This version of the form is intended to supercede all earlier versions of the form, and it is anticipated that use of this specific version of the form will be mandated by the IACUC as of September 1998.

Each study involving laboratory animals must be submitted for review by the Institutional Animal Care and Use Committee (IACUC) on the current IACUC Protocol Form. The form currently in use is IACUC Protocol Form, produced August 1998. IACUC Protocol Form is used for any new project, whether departmental, or funded by external sources. Approval is granted for a 12-month period.

DLAR FORM #2 may no longer be used to renew a protocol or to link the approval of a previously approved IACUC Protocol Form to a new grant application being submitted. You can add the new sponsor to your currently approved IACUC Protocol Form, provided that the proposed use of animals is identical, by sending a written request to the Coordinator.
